Excision benign lesions from 3 sites_help needed for correct coding

Anyone please help me to code it correctly.

Provider excised 3 benign lesions. Ist site is rt lower back and the diameter is 8mm to 7cm. Second site is flank and the diameter is 5mm to 4mm and the third site is right mid back and the diamter is 06cm to 0.6 cm.

Not that familiar but I would think 11406, 11406/59 and 11406/59 with appropiate diagnosis for each one.

I am thinking 11406 qty3 lesions in the same anatomical area.

How is the first lesion 8mm (0.8cm) to 7cm. That doesn't make any sense. Code selection is determined by measuring the greatest clinical diameter of the apparent lesion plus that margin required for complete excision. You didn't provide us with this information and I can only assume that everyone is just making the assumption that the total excised diameter was 7cm. Not sure where or how 8mm comes into play.

The fact that you have ranges for your diameters which don't make sense, makes me wonder if you even know how to figure out the correct diameter. The physician should know the exact diameter of the excision he created, there shouldn't be ranges or if there are they should be minimal ranges.
