can we bill both codes together and do we need to use the 25 modifier with this?

85610 is a lab charge. No modifier needed on 99211.

99211 should not be reported in addition to 85610 for coumadin management. You can separately bill 99211 ONLY if there is separate work documented other than the pre- and post work that is part of the 85610 coumadin testing
