Coding for depo medrol injections

I have a physician that is saying he is giving 70 mgs of depo medrol. Do I have to have him clarify which strength he is giving to know how to code this? Many of our Orthopedic providers we are finding do not document which strength they are giving, only the dosage. (J1020, J1030 and J1040)

You need to know the strength (20mg/mL, 40mg/mL, 80mg/mL) and how many cc's or mL's he is giving the patient. In order for him to give 70mgs he must be giving EITHER 3.5cc of 20mg/mL or 1.75cc of 40mg/mL.

You can't. You need clarification from the provider.
