I have been denied by Medicaid on CPT® J2469 NDC# 62856-0797-01 stating:THE SUBMITTED NDC %1, DOESN'T CORRESPOND WITH THE DRUG CODE, MODIFIER OR DATES ON THE NDC CROSSWALK. Do you know if a modifier is attached to Claim? We are using KX.

NDC# 62856-0797-01 is not the correct NDC# for this drug.

Also, not sure who your Medicare carrier is but Noridian only has 31 polices that make reference to using the KX modifier and they are all in regards to HCPC codes that start with L
