I need codes for private lab tests to report to Medicare and AARP

I have gotten results for powassan virus and it is positive..The lyme's test was negative.
I had a tick bite type disease one week in 1963 with fever, nausea, headache, weakened muscles, and maybe a rash. My parents treated it as a routine child disease. none of the other 8 kids were sick that week.
My other medical issues includes headache, right eye orbit pain, chronic sinusitis, autoimmune mediated neuropathy, C3, C4 protrusion and fasciitis.
