Asymptomatic New Patient

Hello,

My providers are having some difficulty in documenting a comprehensive HPI for a new patient that has no signs or symptoms. A patient will present with an abnormal EKG, Pre-Op evaluation, AFib etc and have no signs or symptoms yet these patients need a full work-up to include a nuclear stress test. Most E&M examples are centered around pain, do you have any examples that would fit one of these scenarios?
