Pre employment visit

If a patient comes in for a pre employment physical can you use CPT® 99455 and V70.5? Been struggling with this all weekend. Thanks,

According to CPT® book- 99455-99456 are codes that we have to use for any work related or medical disability eval services. I used that code for our DOT physical. Pt's ins did not pay for it. I think most ins doesn't pay for it. Pt will end up with a bill.
