annual visit with iud removal

a patient came for an annual visit and wanted an iud removal, should i put a 25 modifer to the visit?

You only mention iud removal so, No. Without a significantly different reason for the visit putting a mod on would be fraud. Sorry, I misread the annual visit part I change my answer to yes.
