Personal History of

Where can I find documentation of when you can start to use personal history of.
Also how long can you use a primary cancer code once a patient has been treated and is just coming in for follow-up. Is there some place I can get written documentation for this. I heard it was 6 months to a year but I would love to have the documentation to go along with this. Thanks in advance.
