Payable codes

Can someone please tell me if Medicare typically pays for the following combination codes or is one considered inclusive?
93459-26 & 59 modifier
36216-59
75756-26 & 59

Why all the modifiers? 93459 is your primary procedure code (I am assuming), there is no CCI conflict with 36216 so no modifiers would be necessary during billing. You cannot bill 93459 and 75756 together, there is no modifier combination that will override the CCI conflict.
