For Medicare if you have 3 units , would you put it like this

CPT® 22851 x 3

or

CPT® 22851

CPT® 22851-59

CPT® 22851-59

please advise.. The mue is 9- so I am thinking the first one is correct but we did it the second way and they only paid two line items.. The other one denied for duplicate.
