Can WC in Illinois be billed for an OV and 99455 on the same day?

This guideline is included for all states - 99455/99456 includes the E/M portion of the eval and the report. 99080 can not be billed separately.

Further clarification:
99455 is reported when the primary treating physician performs the eval/exam.
99456 is reported by a physician that is not the primary treating physician.
