Supervision of Warfarin Therapy

If a doctor sees a patient for a regular office visit, and than also address' their
coumadin/warfarin. Can they add a 99363/99364 along with the visit? If so should they add a modifier? Or can they "upcode" the visit for extra time spent discussing therapy? Do they need to have "seperate" notes reflecting the therapy? Thank you

I believe the codes you mentioned are considered bundled.

Select the level of service based on the history, exam and MDM.
