63030 bilaterally same level with foraminotomy

If I perform bilateral discectomy/foraminotomy (63030), should this be coded as:

63030 Mod 50. Or
63030 RT and then again as 63030 LT

Is there a difference in reimbursement either way?

Code 63030 for a lumbar discectomy [laminotomy (hemilaminectomy), with decompression of nerve root(s), including partial facetectomy, foraminotomy and/or excision of herniated intervertebral disc, 1 interspace, lumbar]
Assign 63030-50 (unit-1) for Medicare (Private payers may do otherwise but CPT® tells you to use modifier -50 as well).

Modifier 50 (Medicare) reimburses 150% of fee schedule allowable for code 63030.
