Tailor's Bunion 5th Toe

I need help with this procedure--Reverse Austin with 1.5x16mm screw--4 cm linear incision placed over 5th MPJ. Dissection carried down to joint capsule where a T capsulotomy was performed. All periosteal & capsular tissues dissected from head of 5th MPJ. The lateral eminence was removed with oscillating saw. A tempered K-wire was inserted at the apex of future osteotomy site. Osteotomy was then cut. The capital fragment was translocated medially about one-third of the width of the metatarsal shaft. A 1.5 x 16 mm screw was placed across the osteotomy site going from the dorsal distal to the plantar proximal direction & tightened to two finger of tightness. The remaining lateral eminence was then transected with a high speed saw. I have looked at CPT® 28308, 28110 or 28296???
I really don't know what to use. Can anyone help!!

Ostectomy means removal of a portion of bone. Excision codes can include ostectomy codes, not to be confused with osteotomy codes. These codes consist of excising bony prominences or sections of bone either partial or complete. 28110—Ostectomy, partial excision, fifth metatarsal head (bunionette). CPT® has listed and identified toe modifiers that can be applied when performing surgery on different toes. These modifiers are:
T1–Left foot, second digit;
T2–Left foot, third digit;
T3–Left foot, fourth digit
T4–Left foot, fifth digit
T5–Right foot, great toe
T6–Right foot, second digit
T7–Right foot, third digit
T8–Right foot, fourth digit
T9––Right foot, fifth digit
TA–Left foot, great toe.
