Primary Exception rule

I read the article here on the primary exception rule, but I'm still a little confused. What qualifies as a "teaching setting"? We have several primary care clinics, some rural, that have med students and residents come in on rotation. The hospital we are affiliated with is NOT a "teaching" hospital.
I know the rules for students...but an still confused about resdients and what they can do, and what we need to do in order to bill with the GC or GE modifiers....can we just do so, or do we need to apply for some kind of special designation??

Primary care exception rule

If your site is not part of a teaching hospital, you can't use the primary care exception rule. In that case, you need to follow the teaching physician rules for the type of service (E/M) for residents. Medical student services are not reimbursable.
