Lesions .5cm or less

When billing for lesion removals and intermediate repair if a lesion is >.5cm or less intermediate repair was is not billed. But what happens if the margins bring it above the .5cm?

Do we bill intermediate repair based upon the size of the lesion (in this case we would not bill) or after the margins are taken (which would allow us to bill intermediate repair).

This is a very common scenario.
