I looked at the 2012 fee schedule. What does it mean when the "#" is next to the code (i.e. # 99214), and then the same code repeats without the "#" sign?

The # means that these amounts apply when the service is performed in a facility setting - such as outpatient. The amounts without the # are for when the POS is the office.
