E/M, Annual Physical AND a procedure, ie cerumen removal

I cant find the right sequence of modifers, Annual Physical is being denied as inclusive. Modifer 25 on the E/M works fine, and removal is paid. Any ideas what modifer to use on the Physical? I have tried an additional mod 25 on that, still rejected.
