medicare/medicaid crossovers for 2 services on same date of service

two services that took place on the same day, provided by two different providers must go on one claim to Medicaid listing only one of the providers (ideally the provider who provided the higher weighted service as per DOH), what do we do in the cases where claims are crossed over from an insurer (such as medicare) that requires two separate claims for each provider, to Medicaid?"
