I am trying to code a surgical code for an angioplasty with stint replacement. im dumb founded. please help. im new at all this

Definition:

Revascularization procedures are used to treat lower extremity occlusive vascular disease by reinstating circulation to the extremity. These endovascular procedures are performed percutaneously or through open surgical exposure and may include balloon angioplasty (e.g., low-profile, cutting balloon, cryoplasty),atherectomy (e.g., directional, rotational, laser), and stenting (e.g., balloon-expanding, self-expanding, bare metal, covered, drug-eluting).

Iliac Vascular Territory: The iliac is divided into three vessels: common iliac, internal iliac, and external iliac. Femoral/Popliteal Territory: This area includes the common femoral, lateral circumflex, profunda femoris, medial descending, lateral descending, perforating branches, superficial femoral, popliteal, and geniculate arteries.



Tibial/Peroneal Territory: This area includes the common tibio-peroneal trunk, anterior tibial, peroneal, and posterior tibial.

History and Physical: A full history and physical should be obtained from the referring physician detailing the physical findings and symptoms that indicate occlusive vascular disease, and the diagnostic test(s) performed that indicate the endovascular revascularization is necessary.



Final Note/Summary: The final note should indicate the detailed description of the procedure(s) performed and any resultant complications. Follow up and treatment options should also be documented.

Indications / Associated Pathology:

Atherosclerosis of the lower extremity arteries

Stenosis or ischemia of lower extremity arteries

Chronic occlusion of lower extremity artery

Intermittent claudication, rest pain, ulcers, gangrene

Diabetes with peripheral vascular disease
