Coding for total contact cast and cast supplies in a Skilled Nursing facility

If a wound consultant physician see a patient in the skilled nursing facility, as ordered by the primary physician and applies a total contact cast CPT® 29445 in a skilled nursing facility, do they use the Nursing Facility Care codes CPT® codes
(99304-99310) or the procedure code 29445?

During the skilled Part A stay, tare the Q codes for the casing supplies billed? If so, are they part of the Part A payment?

If the patient is not under a Part A stay, does the consulting physicain, when reapplying a total contact cast (CPT® 29455) use the procedure code or a subsequent Nursing Facility Code? How are the casting materials billed. Can the SNF or Nursing facility bill them to Part B and collect payment for them?
