Denial

We are billing for an ASC. We coded 23470RT, 29819RT, 64415 59RT, and C1776.

Bill review denied all codes except for the primary code 23470 RT. Stating the rest of the codes are considered incidental or packaged, or bundled into another service or APC payment.

Any help would be much appreciated.

Here is what I got from my coding software regarding code 64415:
CCI edit: This procedure code can never be reported together with code 23470 due to CCI column 2 code edit
CCI edit: This procedure code can never be reported together with code 29819 due to CCI column 2 code edit.

It also says this regarding the RT modifier:
Bilateral modifier 50 might be required based on documentation.
