coumadin

My employer is attempting to set up a coumadin clinic in a small rural practice. We have a PCP in house 2 - three times weekly at this time. (recruiting for the full time position) .
My question is, can we bill a 99211 for the visit if there is no doctor in house? there will be strict policies re: testing and resulting. Medical provider will always be immediately available by phone at all times. thanks

My understanding is a physician must be present in order to be paid by Medicare.

A doctor must have a face to face visit to be able to bill you also have to have documentation to prove the visit was medically necessary

A physician or nurse practitioner must be in the office to bill the 99211. You can not bill 99211 if you are reading results over the phone to the physician. If you bill the 99211 you bill charge under the physician/nurse practitioner who reviewed/singed the results.
