est E/M code visit with parents for a 19 yo female not present

how do you code a visit with parents of a 19yo female (not present) at the request of the parents? We are a Pain Management practice.

She is an established patient.

I don't think I would bill for it.

Or, bill the parents.

Search the archives for family meetings, but since the patient is 19, I wouldn't bill the patient's insurance.

If documentation supported, I would bill the encounter based upon time. “…when counseling and/or coordination of care dominates (more than 50%) the physician/patient and/or family encounter, then time may be considered the key or controlling factor to qualify for a particular level of E/M service. This includes time spent with the parties who have assumed responsibility for the care of the patient or decision making whether or not they are family
members (eg, foster parents, legal guardian).” The following documentation must be notated:
1. Counseling topics and/or how time was spent coordinating the patient’s care
2. Total encounter time***
3. Total counseling and/or coordination of care time
