documentation on new imm adm codes 90460 & 90461

My understanding of these new codes is that the physician should document that counseling of risk and benefits was done on EACH component of the vaccine
Example:
Received dose of measles, mumps and rubella vaccine, live
Received dose of DTaP - HiB - IPV vaccine (intramuscular)
Received dose of pneumococcal conjugate vaccine, polyvalent, IM use

Discussed immunization side effects and provided printed material

In this example: "Discussed immunization side effects and provided printed material" is not proper documentation of each component.

Would
Discuss side effects and provided printed material for HIB
Discuss side effects and provided printed material for Diptheria
Discuss side effects and provided printed material for Measles
and so forth......
be the correct way to document these immunizations that were given?
