CPT® please

Dx: Right L5 radiculopathy

Procedure: Hemilaminectomy and microdiscectomy at L4-L5, operating microscope, intraoperative fluoroscopy, endoscope; suturing of the annulus with PDS

Description:

I'm thinking of choosing CPT® 63030 as there is no mention of removing facets and foramina. Please help
