Modifier -25
If a patient comes in for an office visit (E/M) without a scheduled procedure, and the doctor decides to do a venipuncture, EKG, PFT or U/A at the time based upon the diagnoses should I use modifier -25?

The PFT is the only one of those that I use a 25 on the office visit when done together.
