Global billing

I have had an OB patient that went to the ER on 01/15/2012(claim paid no problem) and then again 02/15/2012 both visits were for Preterm labor- antepartum, threatened. The claim for 02/15 was paid until the delivery claim (02/29/2012) was filed then they denied and recouped the money for DOS 02/15/2012 saying it was global.

The patient had twin which I have never had an issue getting 'baby b' paid for until now. The insurance can't explain to me why the bundled the two DOS together but they keep denying my saying it is global, they also can't tell me why they won't pay for 'baby b'.

Any ideas on what I can do?!?!? The insurance is Blue Shield of CA.

I just got off the phone with the insurance they advised me I needed to appeal again with documentation....this will be the second or third time I have filed an appeal and mailed all the documentation in... :(

Please help!!!
