Transitional care management 99495 & 99496

The guidelines state TCM may only be billed once in the 30 days after discharge, and it should be billed at the end of the service period.

So your DOS is the 30th day or would it be the DOS of the face to face visit and you hold that DOS and send it out on the 30th day?
