New PT Modifier
This new modifier is to be used when a screening colon becomes a diagnostic so would you bill 45378/PT or G0121/PT.

PT modifier
The PT modifier is used on the diagnostic code (e.g. 45380, 45385, etc.), not the screening code, 45378 or G0121. It defines a diagnostic service that began as a screening service.

We have our own ambulatory surgical center. Would PT need to go on the facility claim also?
