Patient had a surgery three weeks ago from a Doctor and came to our practice to see our Hand Specialist and our Doctor had to take this patient back to surgery for the same diagnosis (repeat procedure). What modifier would I use on the procedure code? Would this start a seperate global period for this patient? I was thinking modifier 77.

This again requires more information. Are the physicians in same group? A global is for the operating surgeon and any physician, same group specialty. If medically necessary to do a surgical procedure by a different physician, not same group/specialty, the global will relate to the individual physicians. Is there a complication of original surgery, did second physician perform the same procedure code? This is why assigning surgery codes without an operative note is unwise.
