holter monitor reading

A patient presents to the hospital outpatient department on 5-1-12 to have a 24 holter monitor hooked up. On 5-4-12 the physician interprets/reviews the holter monitor. What date would I use for the physician's interpretation (billing purposes) - the date the holter monitor was hooked up on 5-1-12 or the date of the reading which actually took place on 5-4-12?

IMO, it is the date the service was rendered. You cannot bill a service until documented and performed.

I was using the date that the reading took place (5-4-12) but I was second guessing myself!
