Locum Tenen billing, asking again

Asking again and hoping someone will help me as I have a list of charges to put on by a Locum Tenen which is getting longer and longer as I'm not sure about anything other than modifier Q6. My question was:
Does the name and or the NPI of a Locum Tenen go on the claim at all? Or is it only the name and NPI of the Provider they are substituting for that goes on the claim? I know we are to use the modifier Q6 but am unsure of what else is required on a medical claim when billing for a Locum Tenens services.

We bill with our provider adding only q6.As the modifier
