What am I doing wrong?

Morning,

I have billed the following CPT® codes:

37225
36247,xu
75630,26,xu
75710,26,xu

Now my insurance, Humana Dual Plan, is denying the 37225 indicating "This should be billed with the appropriate code for these services"... What am I doing wrong,

Also, they are denying the 75630, as separately billed services/tests have been bundled as they are considered components of the same procedure.

Anyone else having issues, or can you direct me and tell me what am I doing wrong? I am in Virginia.

Send me your email and I will give you the report from encoderpro. I saved it to PDF file. It appears that all your CPT® 7???? codes are mutualy exclusive to your primary code.
