Modifier 24 is for a E&M service during the post op period for a condition not related to teh reason for the surgery. Modifier 25 is for an E&M service that is done on the same day as a procedure or other service.

24 is for an E/M visit when the patient has had a procedure and is still within the global period but the visit is unrelated to that procedure.

25 is for an E/M visit and another service is performed on the same day, such as an ultrasound.

These modifiers are attached to the E/M code.
