coding 69631 with 20926
where do I get the AMA policy that states that you can bill these 2 codes? Does anyone have a sample of a letter to show these are billable together?

Do you have access to CPT® Assistant? You need to check it for July '98, pg 11, March '01, page 10 & March 07 pg 9- there may be some clues there for you. Try modifier XS on 20926. See MLN Matters # MM8863. You can access the MLN matters article on http://www.CMS.gov.
