Documentation/guidelines physical therapy

Some time ago, the documentation/guidelines came out stating the MD needs to see MCR insured patient within 30 days of their initial evaluation, and then something regarding 60 days, etc. however, I can't locate anything regading this? Does anyone have a site or any insight on this issue?

Where can I find documentation guidelines for Physical Therapist and what is required for CMS MD visit note and RX, etc.
