Nurse visit

Chief complaint and history was taken before patient decides to leave. Provider had not been in the room. Can we charge a nurse visit.

Not for Medicare: doesn't meet the criteria of incident to.

I'm doubtful that the service is complete if the patient left. No objective or assessment/plan.
