New CMS 2 Midnight Rule

With the new CMS 2 Midnight Rule coming into effect can anyone confirm if the coder cannot locate both the formal order for admission and the signed certification that satisfies CMS requirements are we to assign an outpatient code because we will not be able to assign an admission code?

I find alot of information on this subject but no coding guideline.
