Hospital Outpatient consultation and discharge codes

Hello,

Can anyone tell me what are the codes for hospital outpatient consultation, follow-up, and discharge.

I code that with 99223 for consultation and place of service as 22 and for follow-up 99232 and place of service 22 and for discharge 99239 for place of service 22, but all were rejected as the procedure code/bill type is inconsistent with the place of service.

Just by stating OUTPATIENT, the consultation codes are 99241-99245 (Non-Medicare).
Code 99223 is an admit initial encounter to an inpatient status. If you do an outpatient consult and then you admit, the POS is inpatient hospital. Need more information on where and what was done by whom.

Our doctor visit hospital for consultation as outpatient and we have coded it with 99223 to Medicare as outpatient, which was rejected. Our doctor had follow-up visit with the patient as outpatient in the hospital and we billed it with 99232 to Medicare, which was rejected. Thirdly, our doctor discharged the patient as outpatient and we billed with 99239 to Medicare and it was rejected.

So can you please tell me which code should i use for Consultation, follow-up and discharge when our doctor visited the patient at hospital as an outpatient. Insurance is Medicare.

The outpatient visit if new pt use 99210-99215 range.
Follow up visit 99212-99215 range. I have no clue what you mean by discharge
