Using Dx 998.2 for surgeries

Can a surgeon bill for repair of intestine in addition to a major surgery code, if he caused the tear himself? Does insurance pay any CPT® for Dx 998.2?

Let's say excision of a tumor or adhesiolysis is impossible without causing tears in the serosa of an intestine..and so, the surgeon has to spend extra time repairing those injuries. Should those tears be coded as Dx 863.30 or 863.50 instead of 998.2(depending on location) and the repair as 44602 -52? Some coders say if the surgeon causes extra repair work, the work is bundled.
