portacath

I recently saw a patient in the office outside of the global period for removal of a portacath (a central venous access device with subcutaneous port) cpt code 36590. I coded a 99213 on tuesday and performed the 36590 the following day on wednesday. Two questions, do I need a modifier for 99213 (?-25) and I can't find an appropriate ICD code. The patient was not having any problems with the port or catheter...he just wasn't getting any more chemo and didn't want the port. I can't find an ICD code for portacath status.

You will not need to modify the E&M code since the 36590 was done on a day subsequent and it is a minor procedure with only 10 global days. I have billed with a V code for history of malignant neoplasm in that situation without a problem.
