we are try to get correct coding for stimulators trial and implants

L8680 x 16 unites for trials

on the L8680 someone told our doctor that this code gets paid on cost my charges of usual and customary

we bill as a asf facilty free standing rev code 490

For trial we bill 63650 with no modifier and only once even if bilateral. 63650 is not a bilateral code. L8680 in units times the number of contacts/electrodes. We state if LT or RT side. We also bill 95972 (3) to program the transmitter (provided the physician does the programming), sedation 99144 (5) or 00630 (depending upon who provides the anesthesia) and, any meds used. We do the procedures in an office setting. Palmetto GBA has two LCD's L32549 (trial) and L33199.
We do not bill any other codes because this is a trial and not a permanent implant. The lead removal is not billed separately. Again is not a permanent implant.
