Billing Antepartum Package Code to Medicaid

I need to bill Medicaid for Antepartum Package code 59426 because the patient transferred her care. My question is what date of service do I enter for these services? Do I use a span date of first antepartum visit to last antepartum visit or just the last antepartum visit date?
