Can you charge an E/M code solely based on RN notes and "discussing" conditions with the doctor over the phone?

As you describe, no. There has to be a medically necessary face-face service incident to MD. The only code an RN can charge is 99211 but not in your case.

thanks. Even in an ambulatory setting (outpatient clinic)? This is what I thougth but wanted to make sure.
