wide excision

Doc removed a lesion in the forehead & sent it the lab (shave biopsy). It was malignant. Pt was advised to come back for a wider excision of the same area. Lab result came back & its a scar- negative for malignancy. Do I have to code the excision of the lesion or bill it out as biopsy alone? Thanks
