Diagnosis Codes for G0446

Has anyone been able to find the covered diagnosis codes for G0446? (Intensive Behavioral Therapy for Cardiovascular Disease) - I called Medicare in August and they were not able to help me and gave my question and contact information to their policy making department to call me back. I never received a call back.

Would love not to have to deal with denials trying to figure out what they are covering...
