When a family Doctor is requested by a surgeon to do a H&P prior to surgery it is required to bill the appropriate New or Established visit. However, do you still need to use the V72.83 (Exam Pre-OP) as a second code and is the referring Doctor still required on the claim?

The primary diagnosis is always "why" you are seeing the patient, so, yes.
It won't hurt putting requesting MD, nothing specific from CMS.

Pre-op

The V code is primary with condition secondary unless instructed otherwise by payer.
