What is the latest CPT® code for Colonoscopy screening converted to diagnostic?
What is the correct order of diagnosis codes? V code then findings or vice versa?

CPT® code such as 45385/45380 with PT modifier for Medicare. Diag is Vcode first followed by findings code. Some of the other payers are accepting the PT such as Blue Cross and Cigna.
